Community Case Management

Serving Cumberland, Hancock, Penobscot, Piscataquis and Somerset Counties in Maine.

Without supports and services, adults with intellectual or developmental disabilities in Maine could be forced to leave their families and live in institutions to get the care they need. Families often need help developing and implementing a care management plan based on each individual and family needs. A plan that can be altered and reassessed as needed. NFI North’s Community Case Managers help to create that plan- We connect participants with any service they need for their health and well-being. We help our clients to set goals and work towards achieving those goals; we want to help them access friends and community so they can live a rich life, independently and in their own community.

To be eligible for this service:
To be eligible for this service you must be an adult, have a diagnosis of an intellectual or developmental disability and have MaineCare coverage.
